Background and Discussion:
Harris County Public Health operates without a comprehensive, centralized case management platform to provide personalized care coordination and management for divisions across the agency. This lack of integration leads in disjointed care, duplicative efforts, and challenges tracking and monitoring participant progress. Additionally, without a centralized system, there is limited ability to readily analyze data across programs for improvement. There is a well-defined need for an in-house case management solution that can streamline processes, improve communication, and enhance overall quality of care delivered by Public Health divisions. As a result, we are seeking consulting services to provide resources and expertise for developing an in-house case management solution.
