Background and Discussion:
The purpose of the IMHS initiative is to improve maternal health outcomes in the U.S. by increasing access to quality, equitable, comprehensive care for pregnant and postpartum people who experience health disparities and have limited access to basic social and health care services. This initiative will work towards achieving this by:
• Fostering collaborative relationships between health care providers, social services organizations/providers, state Medicaid programs, and state and local health departments to coordinate services and improve maternal health outcomes;
• Developing and enhancing maternal health data infrastructure at the maternal health care practice and population levels;
• Assessing the viability of various models that support integrated health services for pregnant and postpartum people; and,
• Identifying lessons learned for future implementation.
Expected Impact:
If awarded, the grant would entail a five-year performance period with budget being awarded annually for up to five years, or $9 million total. Our goal is to complement the programming available in Harris County Public Health (HCPH) by introducing an integrated care model that supports comprehensive care (including clinical, ancillary, behavioral health, and support services) for pregnant and postpartum people who experience health disparities and have limited access to essential social and health care services. Funding allows HCPH to promote high-quality maternity care through coordinated, evidence-based maternity care management to improve perinatal outcomes in the pregnant Medicaid population. This includes utilizing standardized risk assessments; focus on early entry to prenatal care; care coordination by a nurse, social worker, or other health care professional; standardized care pathways; enhanced access; and patient-centered care.
