Background and Discussion:

Through funding made possible by the Centers for Disease Control and Prevention, Harris County Public Health will provide training and deployment of community health workers (CHWs) to response efforts and by building and strengthening community resilience to fight COVID-19 through addressing existing health disparities. The Community Health Workers for COVID Response and Resilient Communities (CCR) Program provides a mechanism in which HCPH will be able to expand its COVID-19 response through community health worker engagement and partnerships with community-based organizations. The purpose of the grant is to build resilience in communities while strengthening ties between community health workers, partner organizations and COVID-19 response efforts. This agreement will allow HCPH to expand community health worker activities in response to COVID-19 by carrying over unspent funds from year 1 into year 2 activities.

 

Expected Impact:

The Community Health Workers for COVID Response and Resilient Communities (CCR) grant will allow HCPH to build on existing COVID-19 response efforts, specifically by integrating Community Health Workers (CHWs) into partner organizations and care teams in order to successfully engage with local public health led actions and target priority populations with increased prevalence of COVID-19 and those disproportionately impacted by long-standing health disparities. Additionally, the grant will allow HCPH to create partnerships across the region with other CHW organizations and strengthen relevant CHW knowledge, roles, and skills of the staff we hire.
