Background and Discussion:
The Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP) help local public health organizations create interventions to improve the health of a community - a community where everyone has a fair and just opportunity to achieve their best health. The CHA-CHIP process aims to improve community health, remove health disparities, and advance toward health equity. The project helps us understand the biggest health problems in the community and how different groups can work together to solve them. Data from the CHA helps create the CHIP, which is a strategic plan to tackle the most important issues impacting Harris County and coordinate community resources. As an accredited local health department for Harris County, HCPH develops a comprehensive Community Health Assessment (CHA) every five years. Both the CHA and CHIP reports must be completed by December 2024 as requirements for reaccreditation with the Public Health Accreditation Board (PHAB). The goal of the CHA is to achieve fair health for all Harris County community members by finding urgent health problems and aligning community resources.
