Background and Discussion:
The acceptance of funds will continue to support Harris County families and provide direct support through integrated support services, and enhance the health and wellness of Harris County women, children, caregivers and fathers. The request for funding acceptance will help improve the lives of the community members served by increasing case management, care coordination, health education and accessibility to services.

Expected Impact:
Public Health Services will continue to support a comprehensive and person-centered strategy for delivering quality care and support to families to reduce infant mortality rates and improve perinatal health outcomes.
