Background and Discussion:
Harris County Public Health seeks 340B pharmacy services from PMQ Group, LLC d/b/a Avita Pharmacy. The Addendum incorporates the Pharmacy Services Agreement and Business Associate Agreement as Exhibits A and B and establishes the order of precedence if there is a conflict with the underlying Agreement. The Addendum also establishes applicable County contractual terms, including provisions regarding indemnification, taxes, public contact, notice, applicable law, and the Texas Public Information Act.
