Background and Discussion:

Harris County Public Health (HCPH) is an affiliate with Prevent Child Abuse America’s Home Visitation Framework for the Maternal and Child Health Program. Known disparities in Harris County for maternal and child health outcomes indicate a need for a comprehensive public health approach.

The Maternal and Child Health Program with a piloted home visiting model has expanded resource navigation of social and health benefits for new and expecting families through a systematic approach of care coordination, prenatal care, postpartum care, and infant-early child development. Healthy Families America provides training to all staff members to acquire the proper skillset to facilitate home visits up to three years for enrolled families.

Expected Impact:

Affiliation with the home visitation framework will provide direct training to the staff in the maternal and child health program and improve assistance to pregnant mothers and infants at risk for severe health complications by:

1.                     Increasing enrollment into available health coverage options

2.                     Increasing # of prenatal and well-child visits

3.                     Increasing # of home visits for health education and access to care
