Background and Discussion:

Harris County Public Health (HCPH)’s Chronic Disease Prevention (CDP) Program has previously worked to expand diabetes prevention and control efforts but has faced challenges in building sufficient capacity for diabetes management services, including program infrastructure, training, and access. Prior initiatives and assessments identified the need for stronger coordination, technical assistance, and data-driven program development to effectively reach priority populations. The University of Texas Health Science Center at Houston (UTHealth) is a public university comprised of academic health science schools, including the School of Public Health. Through collaboration with their Diabetes Prevention and Control Grant Program, CDP will build on current efforts by formalizing roles in program planning, implementation, and evaluation while also supporting accreditation readiness. CDP will integrate the Diabetes Self-Management Education and Support (DSMES) curriculum into their current portfolio of services and have the capacity to provide services to residents living with type 2 diabetes.

Expected Impact:

The associated Services Agreement will allow the university to provide compensation in the amount of $1,520 upon completion of services and deliverables. Approval of this agreement through August 31, 2028, will strengthen HCPH’s ability to deliver coordinated, evidence-based diabetes management services with DSMES through enhanced program infrastructure, technical assistance, and data-sharing capabilities. In partnership with UTHealth, HCPH will improve workflow development and enable increased service delivery by targeting both pre-diabetic and type 2 diabetic populations. While each party maintains its own funding, the partnership maximizes existing resources and is expected to reduce long-term healthcare expenditures by improving diabetes management and preventing complications. Adoption of this item will expand access to DSMES for high-risk populations, improve health outcomes, and advance the County’s chronic disease prevention goals.

Alternative Options:

If not approved, residents will have decreased or limited access to diabetes education and management support, which will decrease their capacity in managing their condition. Public Health will continue only delivering curriculum services to pre-diabetic individuals and will not have the ability to include diabetic residents.
