Background and Discussion:

A new interlocal agreement was proposed following the guidance and recommendations made during June 12, 2025 Commissioners Court for Harris Health System to fund the nurse case manager position that supports the ACCESS Harris Program. This position was previously funded by Public Health Services. The nurse case manager will support the ACCESS Harris County program for no more than 32 hours per month. ACCESS Harris County is an integrated care coordination model that works across County safety net departments and community-based organizations. The program delivers coordinated and integrated services across systems and programs that focus on supporting a client holistically, addressing multiple needs that ensure better outcomes and greater stability. Harris Health System will collaborate with HCPH, as part of the ACCESS Coordinated Care team, to develop integrated care plans and service goals for eligible ACCESS participants.

Expected Impact:

The Nurse Case Manager plays a vital role by conducting comprehensive assessments to understand participants’ medical, psychological, and social needs. This position will assist ACCESS participants by providing support and advocacy to participants and their families through field visits, phone calls, health advocacy, and referrals to other support organizations. The Nurse Case Manager serves as a subject-matter expert in healthcare education, care coordination, and patient advocacy, ensuring that ACCESS participants receive timely, high-quality care and support. This new interlocal agreement comes at no additional cost to the County and ensures alignment with the County Strategic Plan Goal of improving physical and mental health outcomes across all communities.
