Background and Discussion:
The Community Health Assessment (CHA) and CHIP help local public health organizations create interventions to improve the health of a community - a community where everyone has a fair and just opportunity to achieve their best health. The CHA-CHIP process aims to improve community health, remove health disparities, and advance toward health equity. The project helps us understand the biggest health problems in the community and how different groups can work together to solve them. Data from the CHA helps create the CHIP, which is a strategic plan to tackle the most important issues impacting Harris County and coordinate community resources. As an accredited local health department for Harris County, HCPH develops a comprehensive CHA and CHIP every five years. Both the CHA and CHIP reports must be completed by December 2024 as requirements for reaccreditation with the Public Health Accreditation Board (PHAB). Once the CHA was completed, HCPH invited ten diverse organizations to participate in the CHA-CHIP Steering Committee. Steering Committee members supported the design of the CHIP and have committed to implementing activities that address the goals and objectives outlined in CHIP for 2024 - 2029. Based on data from the CHA and prioritization exercises with the Steering Committee, the following three priority issues were selected: Health Insurance and Care Access, Preventative Care, and Mental Health. Subsequently, a series of Steering Committee meetings were held to develop goals and objectives that address the three priority issues. The CHIP report includes the goals and objectives this CHIP cycle will focus on and the supporting data and context that led to the development of those goals and objectives. Defined activities, performance measures, timelines, and community health indicators that demonstrate progress toward achieving objectives are outlined in the supplemental CHIP Implementation Plan, which is subject to change based on evaluation and quality improvement efforts. Implementation of CHIP activities will begin in January 2025.
Expected Impact:
As a countywide strategic plan, the CHIP allows for local organizations on the Steering Committee to collectively define actions to improve health, make policies, and use resources effectively. Increased effectiveness of programming will come from the improved coordination and communication established through these new or solidified partnerships. HCPH will also use this plan to inform future department planning. While three priority issues are highlighted in this CHIP, HCPH encourages any Harris County community member or organization to take the information presented in both the CHA and CHIP to implement their own action plans to address public health issues that align most with their work. HCPH also invites those interested in implementing the goals and objectives outlined in this report to join the department’s efforts over the next five years.
