Background and Discussion:
The Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP) help local public health organizations create interventions to improve the health of a community - a community where everyone has a fair and just opportunity to achieve their best health. The CHA-CHIP process aims to improve community health, remove health disparities, and advance toward health equity. The project helps us understand the biggest health problems in the community and how different groups can work together to solve them. Data from the CHA helps create the CHIP, which is a strategic plan to tackle the most important issues impacting Harris County and coordinate community resources. As an accredited local health department for Harris County, HCPH develops a comprehensive Community Health Assessment (CHA) every five years. Both the CHA and CHIP reports must be completed by December 2024 as requirements for reaccreditation with the Public Health Accreditation Board (PHAB). The goal of the CHA is to achieve fair health for all Harris County community members by finding urgent health problems and aligning community resources. To create a shared vision for health, we need to understand our current state of health. The CHA achieves this by working with various community partner organizations and community members, assessing needs and strengths, and understanding the root causes of unfair health differences. Conduent Healthy Communities Corporation was awarded the contract to complete the CHA with a term length of October 10, 2023 - October 9, 2024, with one (1) one-year renewal option (220433). Due to various challenges and the vendor not meeting expectations for producing high-quality and timely work, Public Health services directed Purchasing to terminate the contract for convenience on August 29th, 2024, and halt work for the last/third milestone of the project. Public Health Services reproduced the analysis and completed the 2024 Community Health Assessment report internally, which will be promoted on the Harris County Public Health (HCPH) website and social media and disseminated to appropriate stakeholders including HCPH staff, community partners, and community members who participated in the data collection process.
Expected Impact:
The CHA provides a complete, holistic view of Harris County's health status, which will serve as a critical tool for Harris County Public Health and local organizations across multiple sectors to better understand the biggest concerns that currently influence a person’s health in Harris County. Completing the CHA initiates Harris County Public Health’s development of the Community Health Improvement Plan (CHIP). The CHIP uses data from the CHA to prioritize key issues impacting Harris County and develop goals and objectives for local organizations to collectively define actions to improve health, make policies, and use resources effectively.
