Background and Discussion:

Through regular touchpoints, Harris County Public Health (HCPH) and TCP have agreed that the partnership has been successful and should continue for the next four years. HCPH will continue to partner with TCP to utilize Community Health Workers to provide application assistance for state and local benefits, connect patients to HCPH programs and services for which they are eligible, educate patients on various public health topics, and refer patients to other available community resources.

 

Expected Impact:

Approval of this amendment will allow HCPH and TCP to continue and expand efforts to promote resources, distribute materials, and educate and engage TCP patients by connecting them to HCPH programs and services based on their individual needs over the next four years
