title

Request for approval of the first amendment to an interlocal agreement with The Harris County Hospital District d/b/a Harris Health to update the scope of work and amend the renewal terms for the Public Health Diabetes Prevention Program.

end

Background and Discussion:

The Diabetes Prevention Program (DPP) at Harris County Public Health is a free lifestyle change program for individuals over the age of 18 with pre-diabetes within Harris County. Harris Health is a community-focused academic healthcare system that is committed to improving the health of those most in need in Harris County through quality care delivery, coordination of care, and education. This partnership began with Commissioners Court approval of the interlocal agreement with a term of October 1, 2024 - September 30, 2025. This amendment will extend the term of this agreement and allow both parties to execute and continue their collaborative efforts on providing diabetes management education to high-risk patients.

Expected Impact:

This amendment will extend the agreement by automatically renewing it for consecutive one-year terms, allowing both parties to continue providing care and education to high-risk Harris Health patients. Modifications include replacing terms and language to reflect current division name and leadership and amending the Scope of Work from the 2024 interlocal agreement.  It will also allow 1) increased coordination of care between clinical and community-based settings for pre-diabetes education; 2) increased accessibility to Diabetes Prevention Program (DPP) services through the reduction of transportation barriers for at-risk patients; and 3) meeting or exceeding DPP’s key performance indicators, including increases in physical activity, A1C reduction, and BMI reduction. Patients who will receive education primarily reside in the following ZIP codes: 77088, 77039, 77026, 77028, 77009, 77099, 77012, 77047, 77506, and 77074; data shows that 43-50% of adults are classified as obese and 37-45% report low levels of physical activity within these geographical areas.

Alternative Options:

If Commissioners Court does not approve this amendment, diabetes prevention services and education for residents at risk for type 2 diabetes may be interrupted or taken away. These patients will have less or no access to affordable diabetes management education.

Additional notes related to the Strategic Plan: This agreement directly aligns with the Strategic Plan, particularly Initiative N2 and N3 under Objective N, by promoting healthier behaviors that reduce the risk of diabetes through lifestyle modifications. The program emphasizes evidence-based nutrition education, increased physical activity, and collaboration among health agencies, ensuring a comprehensive approach. This collaboration also supports the broader goal of reducing obesity and improving health outcomes.
