Background and Discussion:

Legacy Community Health Services, Inc. is a system of Federally Qualified Health Centers throughout Harris County that serve the most vulnerable populations, especially those underinsured or uninsured. Oftentimes, these populations are eligible for benefits they are not aware of that can positively impact their quality of life. They may also benefit from many of the programs and services available through Harris County Public Health but lack awareness of the available services.

Expected Impact:

By partnering with Legacy and integrating Community Health Workers and Resource Navigators within clinic spaces on a regular basis, Harris County Public Health will directly assist and submit applications for federal, state, and local benefit programs such as SNAP, Medicaid, TANF, Healthy Texas Women, Medicare Savings Program, CHIP, WIC, and Harris Health System’s Financial Assistance Program. In addition, Community Health Workers and Resource Navigators will share and connect community members to relevant programs and services available at Harris County Public Health that may support with the community member’s overall wellbeing. Patients will already be seeking care and will not go out of their way to be connected to public benefits or available programs at the County level. This will increase utilization of Harris County services, increase those that receive benefits, and lower overall healthcare costs.
