Background and Discussion:
Request to update the funding source of 12 positions from the GY22 Community Health Workers grant (2601) to Charity Cares (2117), to allow our agency to continue meeting strategic public health goals and community outreach commitments. Our Outreach team, primarily conformed by Community Health Workers, play a vital role in advancing our public health mission by connecting residents to essential services, delivering health education, and supporting prevention efforts.
