Background and Discussion:
Request to create three new positions (2 Case Managers, 1 Access Navigator) in the Community Mental Health grant. These positions are essential to enhancing service delivery by providing consistent follow-up, individualized support, and comprehensive care coordination for individuals experiencing co-occurring disorders and complex mental health needs.
Expected Impact:
These three positions are intended to support Harris County residents with a clinical diagnosis of a mental health condition and/or co-occurring disorders, which may include intellectual or developmental disabilities, substance use disorders, and complex medical conditions. The addition of these roles will enhance access to appropriate behavioral health services for underserved populations by improving the efficiency of service delivery and strengthening case management capabilities.
