Background and Discussion:

Harris County Public Health (HCPH)’s Chronic Disease Prevention (CDP) Programs provide evidence-based health education and services to residents through partnerships with community-based organizations. The City of Houston’s Houston Public Library (HPL) System has been identified as a valuable community partner due to its accessible locations and established presence in the community. This Partnership Agreement will formalize the collaboration and establish a framework for CDP to provide programming and support at designated library facilities, while HPL will support with space, equipment, accessibility, and promotion of services. This agreement is needed now to strengthen community outreach and increase access to preventive health education and resources.

 

Expected Impact:

HCPH will expand its ability to deliver chronic disease education and services through accessible, community-based library locations throughout Harris County. This partnership will leverage existing County program staff and City library resources, allowing HCPH to broaden its community reach without requiring additional staffing or operational resources. By increasing opportunities for residents to participate in diabetes prevention, nutrition education and counseling, and tobacco and asthma control programming, this partnership is expected to improve health knowledge, promote healthier behaviors, and help reduce chronic disease risk by increasing access and reach among vulnerable populations.

 

Alternative Options:

If not approved, residents will have decreased or limited access to chronic disease education and management support. Delivering services at Houston Public Library locations will not be possible and residents will have to seek services elsewhere.
