Background and Discussion:

The Texas Healthy Communities (TXHC) Program empowers Texans to improve their health in all the places they live, work and play. The TXHC Program helps communities to assess their environments and public health policies and empowers them to make positive changes within their communities. Change happens through local policy development and systems change. Established local advocates drive local decision-making to improve health. A key component of the TXHC Program is an assessment that looks at eight health indicators within the community.

Expected Impact:

Since 2014, 32 communities have participated in the TXHC assessment and have earned recognition at the Gold, Silver, Bronze, and Honorable Mention levels. The TXHC Program has stimulated interest among communities to assess how implementation of system and environmental changes can help reduce the burden of chronic disease. Due to these assessments, communities have identified gaps in their services and have sought to implement appropriate evidence-based programs, policies, and environmental changes to eliminate the gaps.
