Request Summary (Agenda Caption):
title
Request by Public Health Services for approval to accept from the U.S. Department of Health and Human Services grant funds in the amount of $497,200, with no required match, for the FY2025 Healthy Start Initiative: Eliminating Disparities in Perinatal Healthy Program.
end
Background and Discussion:
Through the Healthy Start Initiative, Harris County Public Health will continue to support a comprehensive and person-centered strategy for delivering quality care and support to families to reduce infant mortality rates and improve perinatal health outcomes. The positions will provide critical resources to expand Public Health’s maternal and infant health services and programming, with targeted outreach to families in 14 zip codes with the county’s highest infant mortality rates.
Expected Impact:
This grant would support Harris County Public Health’s health promotion strategic direction, specifically Objective 3.2.4 to develop a multi-stakeholder approach to reduce maternal and infant mortality and promote healthy starts for babies. Our goal with the funding is to complement the programming available in Harris County Public Health by reducing infant mortality rates in Harris County and decreasing disparities in infant mortality across families by achieving steeper declines for groups with the highest IMR. This funding would allow Harris County Public Health to increase case management, care coordination, and group-based and parenting education to facilitate access to medical care and community-based resources; and increase uptake of healthy behaviors before, during, and after pregnancy.
