title

Request that the County Judge execute a services agreement with The University of Texas Health Science Center at Houston to implement the Diabetes Self-Management Education and Support services to Harris County residents through August 30, 2026.

Background and Discussion:

Harris County Public Health (HCPH)’s Chronic Disease Prevention (CDP) Program has previously identified gaps in access to Diabetes Self-Management Education and Support (DSMES) services, particularly among priority populations in areas with limited program availability. The University of Texas Health Science Center at Houston (UTHealth) is a public university comprised of academic health science schools, including the School of Public Health. This partnership will build on these efforts by supporting program implementation, training, and accreditation readiness, and approval is needed to secure technical support for application submission, staff training, and participant incentives, which are critical for launching DSMES cohorts. The agreement outlines the scope of work and services to be delivered through the partnership, including compensation provided by the University for DSMES application/accreditation fee and staff training fees in the total amount that shall not exceed $1,520.

Expected Impact:

The University will provide compensation in the amount of $1,520 upon completion of services and deliverables. HCPH will have the ability to implement and sustain DSMES programming by expanding access to evidence-based services for priority populations in underserved areas. Through collaboration with UTHealth, CDP will strengthen staffing capacity through training, support accreditation efforts, and enhance program delivery through structured cohorts and partner engagement. While the fiscal investment from UTHealth is minimal and primarily supports training and application costs, it is expected to reduce long-term healthcare expenditures by improving diabetes self-management and preventing complications. CDP will integrate the DSMES curriculum into their current portfolio of services and have the capacity to provide services to residents living with type 2 diabetes. Adoption of this item will improve health outcomes, increase program reach, and enhance the County’s ability to deliver high-quality chronic disease prevention services to residents most in need.

Alternative Options:

If not approved, residents will have decreased or limited access to diabetes education and management support, which will decrease their capacity in managing their condition. Public Health will continue only delivering curriculum services to pre-diabetic individuals and will not have the ability to include diabetic residents.
