Background and Discussion:
ACCESS Harris is a coordinated care initiative within the American Rescue Plan Act portfolio. On July 20, 2021, Commissioners Court unanimously voted to establish the ACCESS Harris initiative, and on November 9, 2021, Commissioners Court approved the funding requested to go forth and implement ACCESS Harris. The court also gave permission to solicit bids for a transformative and enabling technology solution that would operationalize ACCESS Harris’ work in the community. Using federally recognized procurement guidelines and with the Harris County Purchasing Department, the ACCESS Harris team conducted a Request for Proposals to select the optimal vendor to provide the enabling technology solution required to put ACCESS into motion. This proposed enabling technology solution is funded by the American Rescue Plan Act (ARPA) and has been reviewed and approved by the ARPA budget steering committee.
The existing contract must be amended, as the Watson Care Manager module will be decommissioned on January 2, 2025, and will be replaced by IBM Care Manager.
Expected Impact:
The IBM Care Manager is a care management tool that allows the Care Coordination teams to develop coordinated care plans, establish team member activities, track client progress and outcomes, automates needs assessments, and provides access to key information regarding clients. Functions can include look up and review of client demographic data and other shared data from source systems, streamlined intake, collaboration, annotation and tracking, care planning and management, closed loop referral processing, alerts and notifications, analytics, and reporting. The tool will allow ACCESS team members to address social determinants of health and provide holistic services to the identified ACCESS Harris County cohorts.
Alternative Options:
Not applicable. The amendment is needed for continuation of services.
