Background and Discussion:

Harris County Public Health (HCPH) would like to partner with Texas Children's Hospital, specifically, their Texas Children's Pediatrics section to integrate HCPH services within their hospital. HCPH has been working alongside TCH over the past several years in bringing this MOU to life and both organizations are ready to implement the integration if passed in Court.

Expected Impact:

Court approval will allow HCPH to serve the most vulnerable populations that pass-through Texas Children's. Through their Social Determinants of Health screening and conversations with providers, Texas Children's will route patients to HCPH to be connected to programs and services applicable to that patient/family. Court approval will allow HCPH presence on site at Texas Children's to better understand the needs and support the family in navigation and procurement of HCPH services. This partnership will allow healthcare and public health to converge and work together to best meet the needs of the individual/family from both a healthcare and public health standpoint as Community Health Workers will work directly with families to support in navigating public health and social resources that play a large part in their health outcomes. This would take place on site at Texas Children's before or after their clinic visit at agreed upon times by HCPH and Texas Children's. Families in critical need of HCPH services will have the opportunity to be connected and receive services they otherwise would not have been aware of or receive to improve their health outcomes.
