Request by Housing and Community Development for approval of an amendment to an agreement with Healthcare for the Homeless-Houston to update the Scope of Services (Exhibit A) and Budget (Exhibit B), and revise the Federal Award Information Table (Exhibit G) for the extended period through September 30, 2025 for the Community Development Block Grant-Cares Act project.

Background and Discussion:
On February 27, 2024, May 7, 2024 and January 9, 2025, Commissioners Court approved agreements and an amendment between Harris County and Healthcare for the Homeless-Houston for the provision of COVD-19 testing, follow-up care for those that test positive to COVID-19, and wrap-around healthcare to eligible homeless persons. The agreements and amendment revised the project budget to adequately cover costs and extended the time of performance.
The current amendment will extend the project by six months through September 30, 2025, increasing the number of persons to be served, and increase funding by $214,400.63 in CDBG-CV funds to cover the costs for the extended period. Total CDBG -CV funds now increase to $500,008.85.
