Background and Discussion:
The purpose of this amendment is to update the definition of Eligible Patient, update the Notice and Liaison, one additional contractor identification badge to be issued to HCPH, and updates to Exhibit A titled "HCPH Census Tracts" and the expansion of zip code service areas.
Expected Impact:
The amendment to this interlocal agreement will allow the expansion of the program to include eligible patients within the added zip codes: 77004, 77023, 77030, 77047, 77048, 77087.
